Advance Health Care Directive
You have the right to give instructions about your own health care and to name someone to make those health care decisions for you. In California these instructions are documented in an Advanced Health Care Directive. The statutory form can be found in the California Probate Code Section 4700-4701. Durable Power of Attorney
A Power of Attorney document lets you appoint someone to represent you in all legal and financial matters. That person is your agent or attorney-in-fact.
